To request a work permit, a student must first download the form below, then complete all required sections on the Employment Permit Application for 14 to 17 Year Olds form and bring it to the AHS School Counseling Office. The Counseling Office secretary can issue a Work Permit once the Employment Permit Application is completed and submitted.

During the summer months, the AHS Main Office or Central Office will issue Work Permits. Please call the office ahead of time at 508-881-0177, to make sure someone is available.
